Athletes, cheerleaders, parents, guardians: Final Forms is now active. You must complete to be eligible for athletics & cheerleading: 1. Student must complete. 2. Parents/guardians must complete. 3. Must turn in a current physical to their coach. Current is anything dated after April 15 2026. If parents, guardians, athletes or cheerleaders do not have an account: instructions can be found on this website under the Athletics tab, then documents.
